AI at Work: An Employee Survey Template

A free 16-question survey for finding out how employees already use AI, where it could help, and who could help lead adoption. Includes a printable PDF.

Before a company picks an AI pilot, it helps to know where employees already are. Some people use AI every day, often on tools the company never approved. Others have not started, or do not trust the output.

In Why CHROs Should Help Lead AI Transformation, HR executive Chris Linford recommends starting with a simple survey. It reveals hidden AI activity, shows where people need support, and gives HR, IT, and company leadership a shared starting point.

This template follows the approach Chris describes. It takes employees about 3 to 5 minutes, and you are free to use and adapt it.

How to use it

  • Copy the questions into the form tool your company already uses, such as Google Forms or Microsoft Forms.
  • Swap the tool list for the AI tools that matter at your company.
  • Tell employees why you are asking and how the answers will be used. Honest answers depend on trust.
  • Look for patterns by department: who is already using AI, which repetitive work comes up most, and who volunteers to be an AI Champion.
